So, when I have missing crew, that just means... what? Their ratings were terrible so they got dropped? It can't be that easy, or people would sabotage their own ratings to get sent home.

That depends on how reliable the technology is. Walking is safer than falling, but air travel is safer than ground travel. Part of that is because it has to be - the consequences of failure are too high, and the ships too fantastically expensive to easily replace, that if it couldn't be done safely, nobody could afford to do it at all.
